Fixes:

- MPI performance enhancement: #97601 enigma bit
- Wrong UAR mapping in pages larger than 4K: Blueflame did not work on UAR pages larger than 4K
- Congestion Notification Packet may cause HCA to stall under extreme conditions
- When in unsolicited mode a completion message is not sent on every completion
